The American Heart Association (AHA) has released a new scientific statement on caffeine and cardiovascular health, providing updated guidance on safe consumption levels and highlighting the differential effects of various caffeine sources. The statement, published in the journal Circulation, concludes that for most adults, consuming up to 400 mg of caffeine per day—equivalent to no more than 5 cups of caffeinated coffee—is safe and may even reduce the risk of several cardiovascular conditions.
According to the statement, regular consumption of caffeinated coffee without added sugars, flavors, or cream is linked to a lower risk of type 2 diabetes, heart disease, stroke, heart failure, and certain irregular heart rhythms. However, the benefits appear to diminish with additives like sugar and cream. The statement also notes that higher caffeine intake, particularly from energy drinks and shots, is associated with increased blood pressure and arrhythmias.
“Caffeine consumed in coffee is a key part of daily life for millions of people, and in our review of the most recent research, for most adults, intake of up to 400 mg of caffeine/day, the equivalent of up to 5 cups of caffeinated coffee per day without added sugars or fillers, is safe and does not increase cardiovascular risk,” said Dr. Gregory M. Marcus, chair of the statement writing group. “However, high doses of caffeine, such as those found in energy drinks including energy shots, may have harmful effects on the heart and should be avoided.”
The statement reviews evidence on caffeine’s impact on blood pressure, cholesterol, heart rhythm, and disease outcomes. For example, drinking 1–3 cups of coffee daily was associated with a higher risk of developing high blood pressure in people with optimal blood pressure, but more than 3 cups was linked to a lower risk. High doses from energy drinks significantly increased blood pressure, especially in those with existing hypertension.
Regarding cholesterol, unfiltered coffee—such as espresso, French press, and Turkish coffee—contains cafestol, which can raise LDL cholesterol. Paper-filtered or instant coffee does not have this effect. The statement also notes that moderate coffee consumption (1–3 cups daily) does not increase atrial fibrillation risk but may be associated with more premature ventricular contractions. Very high caffeine doses from energy drinks have been linked to arrhythmias in young, healthy adults.
The AHA emphasizes that individual responses to caffeine vary based on genetics, metabolism, age, and medications. “There is no ‘one-size-fits-all’ strategy for safe caffeine consumption,” Marcus said. The statement calls for more research on caffeinated products beyond coffee, such as energy drinks, and highlights the need for randomized controlled trials to clarify causal relationships.
The AHA’s findings align with its 2026 Dietary Guidance, which did not issue specific caffeine recommendations due to insufficient data.
